Transmission Electron Microscopy Improves the Diagnostic Sensitivity in Nonbacterial Etiology of Severe Pneumonia: A Retrospective Study

Background: Severe pneumonia is responsible for great mortality and morbidity worldwide, and early-applied effective anti-infective therapy can improve the prognosis of patients. However, identification of infectious agents in severe pneumonia remains a major challenge so far. In this study, the potential utility of transmission electron microscopy (TEM) in detecting non-bacterial pathogens in patients with severe pneumonia was retrospectively evaluated. Materials and Methods: A total of 106 patients diagnosed with severe pneumonia at our hospital from September 2015 to December 2017 were included, and their baseline clinical characteristics were collected. Nonbacterial infectious agents detected by TEM in bronchoalveolar lavage fluid (BALF) and serological tests were summarized. The detection rates were further compared between TEM and serological tests. Results: BALF examination under the transmission electron microscope revealed 24 viruses, 16 mycoplasmas, 18 chlamydia, 2 fungi and 74 bacteria in 99 samples, among which 61 samples were mixed infections. The combined use of serological tests and TEM significantly improved the detection rate of nonbacterial infectious agents in patients with severe pneumonia. Conclusions: Our data support that implementation of TEM could improve the sensitivity for detecting viruses, atypical pathogens and mixed infections in BALF from patient of severe pneumonia. Therefore, TEM may be used as an auxiliary diagnostic method of other microbiological tests in severe pneumonia.

个人简介

李园园,医学博士,中南大学湘雅医院呼吸与危重症学科主任医师,感染学组组长,呼吸重症监护专科主任,副教授,硕士生/博士生/博士后导师。从事呼吸危重症、重症和难治性感染、支气管哮喘的诊治,呼吸介入和治疗,临床工作中具备对呼吸疾病与内科危重症的熟练诊断和救治能力以及支气管镜操作能力。基础研究方向为哮喘的发病机制,临床研究方向为肺部的病原学诊断、耐药菌诊治,呼吸危重症的综合救治体系的建立与实践。

学术任职:中华医学会呼吸病学分会青年学组委员、感染组学委员,中国医师协会呼吸医师分会青年工作委员会委员、内镜分会呼吸内镜青年常务委员,中国抗癌协会肿瘤呼吸病学专业委员会委员,湖南省卫健委高层次人才医学学科青年骨干人才,湖南省卫健委疾控处呼吸疾病防治办公室副主任,湖南省预防医学会呼吸病预防与控制专业委员会副主任委员,湖南省防痨协会理事会副理事长,《中国感染控制杂志》常务编委,《Infectious Medicine》青年编委。

学术成果:主持国家自然科学基金项目2项、省级课题3项,参与并担任多项全国多中心临床研究项目PI。以第一作者或通讯作者发表SCI论文27篇(JCR1区12篇,其中BMJ-British Medical Journal 一作1篇, IF=105.7,European Respiratory Journal共一1篇,IF=34)。获得湖南省科技进步奖3次、中南大学湘雅医院医疗新技术成果奖多次,多次在国际国内学术会议上做专题报告与壁报交流。
