Abstract:The interleukin-1 (IL-1) superfamily mediates critical inflammatory responses. Monoclonal antibodies against IL-1alpha and IL-1beta have transformed the management of autoinflammatory and cardiovascular disease, yet their efficacy is limited by poor tissue penetration, compensatory cytokine networks and immunosuppression. This review traces the evolution of IL-1 therapeutics, examines barriers restricting conventional immunoglobulins in necrotic and fibrotic tissue, and discusses bispecific dual blockade and the shift toward single-domain antibodies with stimuli-responsive nanocarriers.
