Abstract:The assessment of chronic illness care（ACIC）was developed to assess whether chronic illness care provided is in alignment with the Chronic Care Model（CCM）. Till now，it is still one of the most practical，applicable and scientific evaluation tools to compare chronic illness care level among different institutes and evaluate the effect of intervention studies on chronic illness management. The original scale in English has been translated into various languages and applied in many countries and regions.
